Patient Service Coordinator (PSC) Responsibilities

o Respond to all calls and messages. Answer calls live 90% of the time.
o Provide high level customer service to patients, caregivers, practitioners, home health agencies and communities.
o Comply with HIPPA laws and document effectively and appropriately in patient charts/medical records.
o Coordinate care/communications between all involved parties (patient, MPOA/caregivers, Communities, Home Health, vendors (DME) and communities.
o Primary point of contact for practitioners
o Practitioner Scheduling (scheduling to contracted numbers, managing cancellations, verifying insurance and credentialing; recording and managing mileage)
o Processing Specialty Referrals, maintain Specialty Referral Sheet.
o Processing Home Health Orders including Start of Care, change in care or discontinuation of Home Health
o Facility Communications and relationship management
o Discharging Patients
o Sending warning letters as needed.
o Completing and managing Documents with the patient's medical records.
o Process transportation requests
o Work with intake to ensure timely scheduling of New Patients.
o DME processing using both the EMR and GoScripts software.
o Assist Clinical Coordinator with processing other orders such as labs and imaging; provide support for your CC and work as a team to ultimately support the practitioners/patients.
o Take fax processing shifts, 1 or 2 shifts per week as designated.
o Processing medical records requests
